Interest on bank deposits should be included. Accumulated dividends paid by life insurance companies should be included. Do not include interest on tax-free covenant bonds or dividends from money market funds.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Box 2: Refers to early withdrawal penalty.
